When families first sit down with our services coordinator, they complete a one-on-one structured assessment of their child and family's needs. This process helps family members identify areas they need help in so that we can provide individualized and appropriate assistance. A Family Action Plan is then created and given to the family. The Family Action Plan serves as a map and includes information, resources, services and referrals for the family to use.

Needs Assessment

The Needs Assessment is a one-on-one interview with our program coordinator to help parents identify their areas of need and provide assistance.  During the assessment, we look at several areas that contribute to family adaptation to special needs, including:

  • Special needs characteristics
  • Individual and family functioning
  • Formal supports and services
  • Social supports

Custom Family Action Plan

The Family Action Plan is provided to family at the end of the appointment and summarizes the recommendations based on the needs assessment.  It:

  • Summarizes the information, resources, and referrals indicated in the Family Needs Assessment.
  • Serves as a blueprint to help families find and access the information and supports they need.
  • May include a recommendation to receive other COMPASS Services such as Counseling, Family Guidance and/or Education Advocacy.
